Berwick Dental Centre Privacy Policy
This policy lets you know how we treat your personal information that we collect and receive about you. Broadly speaking, personal information is information about you that is identifiable as relating to you, like your name, address or phone number but can include any information or an opinion about you.
In the course of providing our services we do need to collect personal information and are obliged to observe the national privacy principles contained within the Privacy Act 1988.
How and for what reason do we collect your volunteered information?
It is a policy at Berwick Dental Centre that in order to attend our clinic as a patron you will need to volunteer to complete an up to date personal and medical history form before any treatment or consultation is undertaken within our practice. This is to ensure that we can correctly identify who you are and any medical/dental conditions we need to be aware of that may impact on your dental treatment and or your health.
Once this information is collected in physical paper form in our staff will then upload your details into our computer system that is only explicitly for our practice to see, so as to create an electronic record of your identifiable details which include:
Name, Date of Birth, gender, address, contact numbers, email address and your medical and dental history.
Within this personal electronic record will be your volunteered personal details and a detailed written log of any treatment, information, issues or contact you have with us at Berwick Dental Centre from your initial appointment and any subsequent appointments you may attend. This electronic file can only be internally accessed by our staff or is only otherwise accessible to third parties if it is necessary to refer you to outside healthcare providers such as specialists, GP’s or other health professionals with your consent through a written referral letter from your practitioner at Berwick Dental Centre.
What other ways do we use your internet and electronic communication based information?
Except as required by law, Berwick Dental Centre will not give out personal user data to any third party. However, Berwick Dental Centre reserves the right to contact members of our database for verification of appointment purposes. This is done via an SMS service to confirm appointment dates and times. You will have the ability to consent to this service or not via the medical history form.
Berwick Dental Centre reserves the right to perform statistical analyses of user behaviour and characteristics in order to measure interest in and use of the various areas of Berwick Dental Centres site.
Users also should be aware that non-personal information and data may be automatically collected through the standard operation of Berwick Dental Centre’s internet servers or through the use of server-side cookies. Server-Side Cookies are small text files a web site can use to facilitate the user’s ongoing access of the site and allow a site to track usage behaviour and compile aggregate data that will allow content improvements. Cookies are not programs that come onto a user’s system and damage files.
The foregoing policies are effective as of the 23rd of June 2014. Berwick Dental Centre reserves the right to change this policy at any time by notifying users of the existence of a new privacy statement. This statement and the policies outlined herein are not intended to and do not create any contractual or other legal rights in or on behalf of any party.
